Description of Services: |
The Adult Social Care and Support Team offers emotional, social and practical support to individuals who experience enduring mental health problems with the aim of promoting independent living in the community. We provide assistance with: welfare benefits, budgeting, shopping, accessing social/leisure activities, household tasks, accessing public transport, emotional support, dietary needs and communication barriers. |

Are referrals necessary? |
Yes. All referrals need to come via Community Recovery Teams or other health and social care services. Referrals are only accepted for service users on the Care Programme Approach (CPA) register. |
